Accurate blood-pressure guide and seven-day log

Prepare and record home blood-pressure readings for a care conversation. A single reading is not a diagnosis or treatment instruction.

Use it thoughtfully

Before measuring

Use a validated upper-arm cuff of the right size. Avoid exercise, caffeine and smoking for 30 minutes if feasible; empty your bladder and sit quietly for five minutes.

Position and repeat

Sit with back supported, feet flat and uncrossed, and the bare supported arm at heart level. Do not talk. Follow device guidance and record repeated readings with time and context.

Respond to symptoms, not this form

Emergency symptoms require emergency help regardless of a home number. Repeated concerns belong in a professional conversation.

Your worksheet

A protocol-labelled seven-day printable record that can be kept with the diary or shared by reader choice.

Save is optional and writes only to this browser's local storage. Nothing is submitted to Healthspan. On a shared device, another visitor could choose to load that copy.

Do not start, stop or change medicine because of this log.
